What is The Transitions Center? 

The Transitions Center of Hospice Care Corporation is a community-based program that provides services throughout north central West Virginia. The services of the Center are available on-site, in the home, or in one of our seven offices. We offer support in many forms to anyone who has experienced a loss due to death or other life circumstance. 

Our services include a variety of educational and support groups, individual counseling for children and adults, resource and referral services, school and workplace programs, a children's grief camp, and crisis response. Our team of grief and loss professionals are here to help you or your organization. 

The Transitions Center also provides education, training, consultation and resources to those who work with the bereaved, such as school personnel, employers, health care providers, funeral home staff, religious leaders and the community.
